Minigrant Grantees are required to submit a Minigrant Progress Report upon completion of the minigrant project.

In addition to the reporting requirement, each site is visited by First 5 Kern staff at the beginning of the project to record the condition of the site at the time of the visit. Photos are taken and notations made about the impending project. Minigrant recipients are required to notify First 5 Kern staff at the completion of the project and provide a summary of work done or items purchased with Minigrant funds. A second site visit is then conducted to again take photographs and make notations about the project including results and the impact on children directly affected by the project.

Requests for payment are submitted to First 5 Kern staff with the following information:

• Minigrant request for payment form "Minigrant Request for Payment Form"
• Copy of check register or general ledger (depends on your Accounting Department)
• Copies of all invoices
• Copies of checks, credit card statements, etc.
• Copy of bank/financial institution statement demonstrating link between payment method and amount debited
